provide us with evidence of:
- your identity: eg driver’s licence, passport, birth certificate
- address, (in your name): eg council tax, water, gas or electricity, landline/mobile phone
bill, bank statement
Please note: Driver's licence is not accepted as an address document.
- pregnancy or birth: eg MatB1 form (both sides), NHS prescription charge card,
baby's birth certificate, scan report
An official document with your name and the baby's DOB or EDD or in the case of the NHS prescription card,
a valid till date.
Please note: If you are applying for twins or more, please send the scan
report.
