Please Note: To be eligible for a voucher
  • You should be a resident of a participating borough.

  • You should be expecting a baby or have a baby not older than 18 months.

As well as completing this form please provide us with evidence of:
  • your identity: eg driver’s licence, passport, birth certificate

  • address, (in your name): eg council tax, water, gas or electricity, landline/mobile phone bill, bank statement
    Please note: Driver's licence is not accepted as an address document.

  • pregnancy or birth: eg MatB1 form (both sides), NHS prescription charge card, baby's birth certificate, scan report
    An official document with your name and the baby's DOB or EDD or in the case of the NHS prescription card, a valid till date.
    Please note: If you are applying for twins or more, please send the scan report.

Real Nappies for London may keep these details for up to 24 months and contact you for feedback on the scheme. Information will not be passed to persons not involved in the scheme administration or used for any other purpose.
   
The proper use of the voucher is the responsibility of the person named on it. In the event of fraud, the person registered for the voucher will be contacted by the issuing authority and may be prosecuted.
