As well as completing this form you need to provide us with with evidence of:
  • your identity, eg driver’s licence, passport, birth certificate

  • address, eg council tax, water, gas or electricity, landline phone bill

  • pregnancy or birth: MatB1 form or baby birth certificate.

You can post photocopies (do not send originals) to:

The proper use of the voucher is the responsibility of the person named on it. In the event of fraud, the person registered for the voucher will be contacted by the issuing authority and may be prosecuted.
